

When a patient presents with a new symptom, or when a brief or complete clinical examination is required (e.g. to justify the use of a PRN drug), the history is the first step.
The usefulness of O-PQRSTU: don't forget the minimum data you need to collect. Your notes will be more precise if you follow a method. The subjective data collected will guide the rest of your clinical examination.
The information you provide must be complete and thorough.
In the first part of the clinical examination, i.e. the anamnesis or health history relating to a dominant complaint, the O-PQRSTU is used to obtain the minimum subjective data.
